Under-Eye Fillers: How to reverse with Hyalase
Under-eye fillers Reversal using Hyalase are commonly requested to correct botched fillers for undeer eye hollows. When done meticulously, they can provide a refreshed and youthful appearance. However, not every under-eye concern can or should be corrected with fillers, and certain cases require surgery instead.
In my 10+ years of experience, I’ve performed thousands of under-eye filler treatments and have also reversed numerous botched filler cases. Here’s what you need to know before considering this procedure.
Why Under-Eye Fillers Require Expertise
The under-eye area is delicate and anatomically complex, which makes filler placement in this area highly technique-sensitive. Even a small amount of filler can lead to overcorrection, creating a puffy or unnatural look. That’s why it’s crucial to work with a provider who has a deep understanding of facial anatomy and aesthetics.
As physicians, it’s our responsibility to educate patients. What you see in the mirror may differ from what photographs show, and it’s important to set realistic expectations for what under-eye fillers can achieve.
Complications and Risks of Under-Eye Fillers
While under-eye fillers can deliver beautiful results, they also come with risks, including:
• Overfilled or underfilled results: Too much filler can create puffiness, while too little may not achieve the desired effect.
• Infections: Though rare, infections can occur and require prompt treatment.
• Nodules, biofilm, or granulomas: These lumps under the skin can be difficult to treat and may require medical intervention.
• Permanent issues: Some complications, though rare, can lead to long-term problems.
Why I Avoid Permanent Fillers
Permanent fillers should never be used under the eyes due to the high risk of complications. I always advise my patients that under-eye fillers should be approached with caution and seen as a one-time solution. If additional filler is needed later, it must be done with careful assessment and precision.
Under-Eye Fillers and Surgery
If you’re considering surgery such as blepharoplasty for under-eye concerns, it’s important to avoid fillers beforehand. Fillers can complicate surgical outcomes, as they often need to be dissolved prior to the procedure. Always consult a plastic surgeon if surgery is on your radar, and avoid getting fillers from non-surgical providers without considering this factor.
Correcting Botched Under-Eye Fillers
Unfortunately, I’ve seen many patients who come to me after receiving botched fillers in the under-eye area. Common issues include overfilling, asymmetry, or improper product placement. Thankfully, hyaluronic acid-based fillers can be dissolved using Hyaluronidase (Hylenex) to reverse these problems.
Correcting botched fillers requires expertise, a deep understanding of complications, and a commitment to patient safety. If you’ve had a bad experience, don’t hesitate to seek help from an experienced provider.
Procedures I Avoid in My Practice
Safety is my top priority, which is why I do not offer certain procedures that I believe carry higher risks or unpredictable outcomes, such as:
• Liquid nose fillers
• Threads for the nose or brows
• Permanent fillers for any area
By focusing on safe, proven treatments, I ensure that my patients receive results they can feel confident about.

Under eye filler reversal
Click the above link on YouTube and watch how Dr.Shanthala is performing an under-eye filler removal for a patient who was regularly injected with both Radiesse and Juvederm fillers by another Provider every six months. She did not realize she did not need these fillers regularly and eventually noticed puffiness and swelling under her eyes. Concerned, she reached out to me for help. During the examination, I observed that the fillers used were not suitable for the under-eye area; they were hydrophilic, meaning they absorb water and cause swelling, and they were not placed correctly. How to Reverse Tyndall effect from Botched fillers?
I meticulously injected the reversal enzyme hyaluronidase to dissolve the Tyndall effect from superficially placed HA fillers, avoiding the vein near the medial canthus. The patient did not suffer any bruising during this process. I also explained to the patient that additional filler could be removed if necessary after the initial filler removal. You will see this in her before-and-after photos on our social media.
